Intake Protocol
A standard OzHelp support service referral form is completed either by the referring agency/organisation or by a counsellor when the referral is self-referred.
Referrals are assessed at a weekly intake meeting for eligibility and appropriateness.
Referrals are assigned to counsellors or re-referred to a more appropriate agency.
Client Consent
A client consent form must be completed and kept on file. The consent informs of who can be contacted in relation to appointment changes etc. and grants permission for the exchange of information with other sources if required.
